Two Ways to Remove Biofilm & Plaques
The oral cavity is a sea of micro-organisms normally living in balance with the entire body. When this balance is lost, the pathogenetic (disease-causing) micro-organisms become dominant and create what we call biofilm. This biofilm supports different types of infections such as bacteria, fungi, yeast, viruses, and parasites. There are two main ways to remove biofilms and plaques:
Conventional Methods
The traditional way to remove biofilm and plaques is the mechanical removal of biofilms plus the use of drugs such as antibiotics with the consequence of drug side effects. In addition to drug side effects, we need different drugs to eliminate different types of biofilms such as bacteria or fungi and their dominance. After a certain time, our body becomes resistant to antibiotics and the same medication will not work.

Types of Dental Cleaning
There are different types of professional teeth cleaning, and the type that our hygienist and Dr. Horiyat are likely to utilize relies heavily upon your specific oral care condition.
- Prophylaxis Cleaning (Regular Cleaning)
A prophylaxis cleaning is mainly used on individuals with an overall healthy mouth and is designed to perform routine maintenance and prevent dental problems such as gum disease or bone loss. During a prophylaxis cleaning, plaque, tartar, and calculus (harden plaques) are removed from both sides of teeth as well as from in between them. Dental prophylaxis cleanings are generally recommended at least twice a year to keep your teeth and gum healthy; however, high-risk patients may require them more.
- Periodontal Scaling and Root Planing (Deep Cleaning)
Scaling and root planning is non-surgical deep cleaning of the gum, gum lines, and other supporting structure of teeth; it is performed on patients who are diagnosed with gum diseases such as gingivitis or periodontitis. Deep Cleaning is used to treat gum disease and promote the reattachment of receding gum. There are two main steps to deep cleaning: the first step is called scaling in which plaque and tartar are removed from the visible surface of the teeth as well as from the gum pocket below the gum line. The second step is known as planning in which the root surfaces are smoothed. This planning step will make it easier for the gums to reattach themselves to the teeth.
- Gross Debridement
This type of cleaning is performed when a regular teeth cleaning is not enough. It is usually used for people who have put off dental visits or teeth cleaning for a while and have a significant buildup of plaque and tartar. During a gross debridement, plaque is first removed with a scaler tool. Since tartar is hardened, a special electric tool is usually required to loosen and remove tartar from the teeth. Once this is complete, a regular prophylaxis cleaning will be performed.
- Periodontal Maintenance Therapy
Periodontal maintenance cleaning is recommended for those who had a history of gum disease; it helps patients to maintain healthy teeth and gums long-term. Periodontal maintenance cleaning involves a careful inspection and a thorough cleaning to prevent gum disease. This includes:
- Inspection: Teeth x-rays & visual examination of the mouth
- Subgingival Cleaning: Calculus is removed along and under the gumline.
- Supragingival Cleaning: Dental scaling tools are used to clean the area of teeth that is visible above the gum line.
- Root Planing: The root of the teeth is gently smoothed to remove any remaining bacteria that persists.
