At the beginning of your visit, you should describe all the signs and symptoms that you’ve notices such as snoring, fatigue, general sleepiness, lack of concentration, insomnia and any other symptoms. You also need to include your medical history such as a family history of sleep apnea or any other kind of sleep disorders, risk factors such as hypothyroidism, deviated septum, obesity, age, and lifestyle, smoking or alcohol habits, and any related health conditions.

During your visit and oral examination, we will look for physical factors that might contribute to sleep apnea episodes and airway blockage such as lip/tongue ties, upper and lower jaw size, tongue size & positions, size and shape of your palate, being overweight, posture, and neck circumference. Men with large neck circumferences of more than 17 inches and women with more than 16 inches are more likely to have issues with their breathing during sleep. She may evaluate your “Mallampati Score” by using a quick tool to measure the amount of space in your oral cavity. She may also recommend our in-house cone beam CT scan to evaluate your airway, facial structure, and sinuses.

We provide onsite 3D imaging systems to visualize your teeth, jawbone, and airways structures.

Sleep apnea screening can be performed using Cone Beam CT-Scan (CBCT) by visulalizing the anatomy of the upper airways. This 3D airway analysis highlights areas of narrowing that may contribute to sleep apnea. Dr. Horiyat, our sleep apnea expert, can use this CT-Scan screening process to identify if any of her patinets is at a high risk of sleep apnea due to the anatomy of their airways. Many of our patient claim that this 3D airway analysis screening process helped “save their life” as it was the first step in  receiving treatment to find out the root cause of their medical condition which was untreated sleep apnean.

If your medical history and physical examination give us reasons to believe you may have sleep apnea, we’ll arrange for an at-home sleep test to be performed. These tests can measure how many sleep apnea episodes you have per night, measure your blood oxygen saturation level, brain activity, heart activity. and level of activity in muscles controlling your breathing.

Dr. Horiyat may suggest visiting your medical doctor to perform additional tests such as blood tests for hormones, and a pelvic ultrasound of the ovaries. She may recommend seeing an ENT doctor to evaluate your allergies, nasal airway, sinuses, and throat conditions and issues. She may also recommend taking an in-house 3D CT-Scan to evaluate your airways and bone structures.

VivoScore is a basic review of a “Sleep Image” report that allows Dr. Horiyat to review many factors that affect your sleep such as AHI (sum number of pausing),  number of hypopneas (period of shallow breathing), RDI (respiratory Disburbbance Index), Sleep Pathology (indicator of pain upper airway resistance and OSA), Sleep Opportunity (duration, latency, Sleep Quality, Sleep Stability, SAI (cardiovascular reaction to paused breathing), and Oxygen Desaturation. Based on this report, Dr. Horiyat can tell if you have sleep apnea or not; however, you have the option to visit 3rd party medical sleep specialist or medical doctor (MD) to perform any pre- and post-evaluation before or after at-home sleep test performed and receive a detailed and formal report.

Oral appliances, also known as mandibular advancement devices, are custom fit devices that are worn only at night that fit much like a sports mouth guard. It supports the jaw in a forward position and is designed to help maintain an open airway.

Advantages of Oral Appliance Therapy

Mandibular advancement splints are by far the most numerous types on the market. They all function to reposition and maintain the lower jaw (mandible) forward in a protruded position during sleep. This serves to open the airway by indirectly pulling the tongue forward.

Dr. Horiyat is a certified provider of the Vivo-DNA Appliance (Daytime Nighttime Appliance) system. She can now treat the teeth, jaw, temporomandibular joint (TMJ), face, and airway for optimal growth and development by engaging the patient’s underlying genetics without the use of surgery with the help of DNA appliance therapy. 

The Aligner Sleep Appliance® (ASA) is an FDA-cleared aligner. The ASA combined with clear aligners enables Dr. Horiyat to simultaneously straighten teeth, fix bite, and occlusion problems, and solve her adult patients’ mild-to-moderate obstructive sleep apnea (OSA).

ASA is worn over the clear aligners during sleep to support the lower jaw in a mild forward position (up to 8mm), by repositioning the lower jaw to create a maximum reduction of soft tissue obstruction in the oropharynx. 

The Herbst oral appliances offer patients both vertical and lateral oral flexibility as well as plenty of comfort. It can be adjusted by the patient for more personal control and a more satisfying fit. 

There are only a few tongues retaining devices available, but they have been well studied and shown to be effective in many patients by means of a suction bulb that hold the tongue. When the tongue is in a forward position, the back of the tongue does not collapse during sleep and obstructs the airway in the throat.

The SomnoDent (COAT) works by gently holding your jaw forward during sleep, which improves the patency of the airway reducing snoring and sleep apneas. It is a comfortable two-piece “mouthguard” that can be adjusted to bring your jaw further forward if needed, and it is fitted by a dentist trained in sleep medicine. There is an increasing body of evidence to show that the SomnoMed COAT is effective because of the high level of patient comfort. Patients wear their SomnoDent on average 7.5 hours per night. While average nightly usage of CPAP in a recent study was 3.3 hours per night.

Narval CC has light materials that allow for free jaw movement and more space for your tongue, meaning that patients can speak clearly and even drink liquids while wearing it.

Is a MAS Comfortable?

The answer is simple — YES! A mandibular advancement device is made to be comfortable so that you can sleep without even noticing you’re wearing it. It does not prevent you from breathing with your mouth open and will even eliminate snoring for patients with sinus congestion or allergies. You may experience some slight stiffness of the jaw for the first few mornings after wearing the device, but this feeling is only temporary and will go away after you wake up and remove the device.

“Myofunctional therapy” might be an unfamiliar term for most people, but it offers a proven, non-invasive way to help reduce and even stop obstructive sleep apnea symptoms by exercising for a few minutes a day.  The main goal of myofunctional therapy is to retain and strengthen the tongue and orofacial muscles to prevent the airway from blocking during sleep. In general, this approach is ideal for children and adults who have or had tongue or lie-tie.

Your sleep doctor may utilize physio-therapeutic equipment such as TENS to relax tense jaw muscles. The Transcutaneous Electrical Nerve Stimulator (TENS) delivers pulses of electricity to your body’s neurotransmitters which helps neurons release endorphins and signals your muscles to relax. To do so, a nerve stimulator is implanted surgically under the skin on the right side of the chest to control tongue movement by stimulating the hypoglossal nerve which will ultimately prevent the tongue from blocking the throat. 

 

CPAP

CPAP (continuous positive air pressure applied through a nasal mask) is the most common and standard form of treatment for obstructive sleep apnea (OSA). The CPAP machine consists of a face or nasal mask that is connected to a pump, providing a positive flow of air into the nasal passages to keep the airway open. This pressure ensures that the airway doesn’t collapse during sleep. CPAP is recommended as the first line of treatment for patients with severe obstructive sleep apnea. Patients with mild-to-moderate sleep apnea can usually choose which therapy they would prefer.

Common CPAP Side Effects and Complaints

Surgery

While not considered the first line of treatment for snoring or sleep apnea, surgery may be an effective option for patients who cannot tolerate CPAP or oral appliance therapy. With many surgical options available outside of Aria Dental, it is up to your surgeon to find where the obstruction is in the patient’s upper airway or nasal passage and determine what the best solution is. Some of the surgery types that could be recommended by an oral surgeon or plastic surgeon are:

A septoplasty straightens your nasal septum (the bone and cartilage separating your nostrils) to make it easier for you to breathe. 

Turbinate reduction addresses the curved bones in the nasal passage, which are called turbinates. The septoplasty can reduce the size of the curved bones to open up the airway.

If the obstructive sleep apnea is caused by a small airway, your doctor may recommend jaw surgery. 

During this type of surgery, the surgeon alters the shape and size of the lower jaw, upper jaw, or hard palate. This type of surgery will help increase the size of the airway and give the tongue more space to rest properly in the mouth, which can eliminate the cause of sleep apnea. 

This procedure repositions the bones in the jaw, creating more space behind the tongue to open up the airway. Full recovery may take about six weeks. Maxillomandibular advancement surgery is highly effective for treating sleep apnea. 

Hypogolssal never is a major nerve that stimulates several muscles in the tongue that are involved in movements that make the tongue protrude (push forward) and retract (pull back). These muscles are genioglossus, hyoglossus, and styloglossus. 

Hypoglossal nerve stimulation (HGNS) is a sleep apnea treatment involving a surgically implanted medical device that stimulates the hypoglossal nerve in a rhythm synchronized with the patient’s breathing to help keep the airway open and clear during sleep.

Uvulopalatopharyngoplasty (UPPP) is a surgical procedure that involves removing tissue from your mouth and throat to widen your airway. The surgeon will remove some of:

Uvula – The soft flap of tissue that hangs down in the back of the Throat

Soft Palate – The soft arch of tissues in the back of the roof of the mouth, right before the throat.

Pharynx – The part of the throat below the sinus and above the windpipe. 

 

The hyoid bone is located below the lower jaw and helps you to speak and swallow. Hyoid suspension is a procedure that repositions the hyoid bone to open up the airway and prevent your tongue from falling back into your mouth. 

Soft palate implants are also known as the Pillar Procedure and treat mild cases of sleep apnea. In this procedure, the oral surgeon implants three small polyester rods into a soft palate. These rods make the soft palate stiffer so the palate does not relax to block the airway while asleep.
