understanding your sleep problem
Sleep Apnea Is More Than Just Snoring.
In the United States, millions of people suffer from undiagnosed sleep apnea but they experience medical conditions caused by this disorder; millions more have been diagnosed with this disorder but have no permanent solution to treat it, thus, they may have to wear an intrusive face mask (CPAP machine), rely on routine oral appliances that are required to wear for the rest of their lives, or get an invasive surgery to trim the throat tissues by reshaping their facial or jaw bones.
To understand your sleep problem and identify a proper obstructed sleep apnea therapy, you need to find out the underlying root cause of sleep apnea. Dr. Maryam Horiyat follows Dr. Weston A. Prices’ theories by understanding that one of the main reasons for sleep-disordered breathing such as sleep apnea is due to obstructive airways caused by the underdevelopment of the maxilla (upper jaw) which provides the floor of the nasal cavity and houses all the nasal sinuses.
Dr. Horiyat and her dental team are trained to recognize the subtle signs of airway problems, work with you to diagnose any disorders, and create a personalized plan to manage or treat breathing problems that work for you. As a biological sleep disorder dentist, Dr. Horiyat could provide both excellent snoring relief and alternatives to CPAP therapy for OSA.
Our practice is uniquely qualified to help you recognize the leading indicators of obstructive sleep apnea. We work closely with you and a team of specialists, including a board-certified sleep physician to arrive alternatives at a proper diagnosis. We design a personalized treatment plan to provide immediate relief and hope for a lasting resolution.
what is obstructive airway
Upper airway problems, also known as upper airway disorders, are conditions that can make it difficult to breathe by narrowing or blocking the upper breathing passages. Upper airway issues often result in snoring and obstructive sleep apnea.
An obstructed airway interferes with having quality sleep which is one of the most crucial factors for overall physiological health and well-being. Without good sleep, the body, mind, and brain cannot perform the reparative processes necessary to function at an optimal level. An obstructed airway can produce lasting effects in the form of daytime inefficiency, fatigue, confusion, forgetfulness, and lack of focus, which affect whole-body health and wellness.
Dr. Horiyat has been providing her patients with modern sleep apnea and snoring oral appliances and laser therapy that may help deliver a quiet and healthy night’s sleep and decrease the risk of developing other medical conditions.
The progression of sleep BREATHING disorders
The airway is key to life. Normal breathing is silent breathing through your nose; it is effortless and quiet. Normal breathing is vital for living a vibrant and happy life and allows our bodies to go into normal deep restorative sleep, and to awake refreshed and ready to take on each new day.

diagnostICS & ASSESSMENT
CONFIRMING THE NATURE OF YOUR SLEEP APNEA
As important as it is to address your sleep apnea as you can, Dr. Maryam Horiyat can only create a treatment plan once she has confirmed that you’re suffering from the disorder. The diagnosis phase allows her to narrow down the kind of treatments that would be helpful in your specific case. If you’ve been feeling exhausted during the day and have been told you snore loudly at night, call us today to schedule a consultation with our dentist sleep specialist and begin the process.
medical history
At the beginning of your visit, you should describe all the signs and symptoms that you’ve notices such as snoring, fatigue, general sleepiness, lack of concentration, insomnia and any other symptoms. You also need to include your medical history such as a family history of sleep apnea or any other kind of sleep disorders, risk factors such as hypothyroidism, deviated septum, obesity, age, and lifestyle, smoking or alcohol habits, and any related health conditions.
physical exam & Initial screening
During your visit and oral examination, we will look for physical factors that might contribute to sleep apnea episodes and airway blockage such as lip/tongue ties, upper and lower jaw size, tongue size & positions, size and shape of your palate, being overweight, posture, and neck circumference. Men with large neck circumferences of more than 17 inches and women with more than 16 inches are more likely to have issues with their breathing during sleep. She may evaluate your “Mallampati Score” by using a quick tool to measure the amount of space in your oral cavity. She may also recommend our in-house cone beam CT scan to evaluate your airway, facial structure, and sinuses.
3D airway analysis
We provide onsite 3D imaging systems to visualize your teeth, jawbone, and airways structures.
Sleep apnea screening can be performed using Cone Beam CT-Scan (CBCT) by visulalizing the anatomy of the upper airways. This 3D airway analysis highlights areas of narrowing that may contribute to sleep apnea. Dr. Horiyat, our sleep apnea expert, can use this CT-Scan screening process to identify if any of her patinets is at a high risk of sleep apnea due to the anatomy of their airways. Many of our patient claim that this 3D airway analysis screening process helped “save their life” as it was the first step in receiving treatment to find out the root cause of their medical condition which was untreated sleep apnean.
sleep studies (Polysomnography)
We facilitate a sleep apnea test in the privacy of your own home.
If your medical history and physical examination give us reasons to believe you may have sleep apnea, we’ll arrange for an at-home sleep test to be performed. These tests can measure how many sleep apnea episodes you have per night, measure your blood oxygen saturation level, brain activity, heart activity. and level of activity in muscles controlling your breathing.
ruling out other health issues
Dr. Horiyat may suggest visiting your medical doctor to perform additional tests such as blood tests for hormones, and a pelvic ultrasound of the ovaries. She may recommend seeing an ENT doctor to evaluate your allergies, nasal airway, sinuses, and throat conditions and issues. She may also recommend taking an in-house 3D CT-Scan to evaluate your airways and bone structures.
Vivoscore
VivoScore is a basic review of a “Sleep Image” report that allows Dr. Horiyat to review many factors that affect your sleep such as AHI (sum number of pausing), number of hypopneas (period of shallow breathing), RDI (respiratory Disburbbance Index), Sleep Pathology (indicator of pain upper airway resistance and OSA), Sleep Opportunity (duration, latency, Sleep Quality, Sleep Stability, SAI (cardiovascular reaction to paused breathing), and Oxygen Desaturation. Based on this report, Dr. Horiyat can tell if you have sleep apnea or not; however, you have the option to visit 3rd party medical sleep specialist or medical doctor (MD) to perform any pre- and post-evaluation before or after at-home sleep test performed and receive a detailed and formal report.
